OPENING TIMES

Boultham Park Medical Practice including the telephone line is open between 8.15 am and 6.30 pm Monday to Friday

 

How to register:
Patients who wish to register must live within the practice boundary, please ask at reception. Click here for a registration form.

To register, patients must bring along their existing medical card or fill in a registration form available from reception. You will need to make a 'new patient check' appointment with the practice nurse. It is essential that you attend this appointment to enable us to evaluate your medical requirements. It helps us check on any potential problems you may have and also helps us to monitor the health needs of the practice as a whole.

Blood pressure, urine testing and general health enquiries will be carried out at this appointment. It often takes some time for your records to reach us from your previous doctor so please take advantage of this service. You will be registered with the Practice, however you will always have the right to express a preference of practitioner. We will not refuse to accept you on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition.

Temporary Residents:
We are happy to see anyone staying with or visiting a registered patient if they require a Doctor and they cannot get to their own practice due to distance.

 

OUT OF HOURS (OOH) ARRANGEMENTS
During the week from 6.30 pm in the evening to 8.00 am the following morning and at weekends and Bank Holidays there is an emergency doctor service available. Lincolnshire Primary Care Trust is responsible for commissioning this service.

The service can be contacted on: 0845 045 0281.

Patients should only ring this number for urgent medical problems that cannot wait until the next day to be treated. The OOH Emergency service is available for an urgent consultation which cannot wait until your GP surgery is open. The service is located at the Primary Care Out of Hours Emergency Service (adjacent to A&E at Lincoln County Hospital).

If you dial the normal practice number of 01522 874444 you will hear a recorded message and be automatically transferred to the Out of Hours Service. IF FOR ANY REASON THIS SYSTEM FAILS and you are unable to contact the doctor and the emergency persists, then ring 999 without delay.

 

What to do in an Emergency:
If you need medical assistance in an emergency please dial 999 and request an ambulance.

A&E:
The nearest Accident & Emergency Unit is at Lincoln County Hospital, Greetwell Road, Lincoln, LN2 5QY. Telephone: 01522 512512.

 

NHS Walk-in Centre:
The nearest to this practice is approximately 3 miles away. The address is 63 Monks Road, Lincoln. The centre provides access to a range of NHS services, including health information, advice and treatment for a range of minor illnesses (coughs, colds, etc) and minor injuries (strains, sprains, cuts). They are open from 8.00 am to 8.00 pm every day excluding Christmas Day and New Year's Day. This is a nurse led service although there is a GP available should you need to see one.

NHS Direct:
NHS Direct offers free expert confidential health information and advice 24 hours a day throughout the year on 0845 4647. You will be charged a local rate charge for the telephone call. Alternatively you can view their website at www.nhsdirect.nhs.uk for high quality reliable health information.

For deaf people and those hard of hearing, a telephone service is available on 0845 606 4647.

 

Dental:
For dental care please consult a dentist, as Doctors are not trained to deal with dental problems.

 

Protected Learning Time:
To maintain a high standard of care, patients can be confident that all members of the practice team receive regular training and updating.

The practice will close the surgery between 1.00 pm and 6.30 pm on the 3rd Tuesday of most months in the year (8 in total) to enable the GPs and staff training outside the practice. During this period the 'out of hours' service will provide GP cover to our patients.
